Getting Started

In order to maximize our time together, please fill out these forms before our initial meeting.  This saves you time in the office and also allows us to have a preliminary plan designed for you starting on your first visit.  We encourage you to complete the forms on line; however, if you would prefer, you are welcome to print them off and scan/email them back. If you would like to fill them out on your computer and email them, please contact us and we will send them in a Word format.

The welcome letter is for you and is simply an introduction to our office and congratulations to you for making the first steps in changing your health.  The intake form and health objectives form is merely a health history.  Please know that the more information we have, the greater we can assist you.  The diet diary is our window into your daily eating habits, as well as your sleep patterns and fitness routines.  Please be as detailed as possible here and be confident that there is no judgment being placed.  Lastly, the disclaimer merely states that Fuel For Life, LLC is here for education and support, and not to treat, diagnose, or prescribe or replace traditional medical care.
